Patient Procurement
PROCUREMENT OF CDCA PATIENTS RESTS SOLELY WITH THE CANDIDATE, as the College assumes no responsibility to identify potential patients. Liability for treatment provided by candidates is the total obligation of the candidate.
Candidates must have screened their patients prior to arriving at the College of Dentistry for the CDCA examination.
College Required Information – patients may not be seated until the following forms are submitted to Clinic Administration by the candidate – failure to comply may result in your exam being stopped for non-compliance
Forms
The following forms are available below or may be obtained in the Clinic Admin Office - Rm. 1130 Postle Hall. EVERY CDCA CANDIDATE MUST COMPLETE AND RETURN TO THE CLINIC ADMIN OFFICE THE FORMS NOTED PRIOR TO ORIENTATION OR NO LATER THAN 7:00 A.M. THE DAY OF THE CLINICAL EXAMINATION.

Only one registration form is required listing all your CDCA patients. This list is to include all patients the candidate anticipates including "back-ups". Place each name on the appropriate line below your name, indicating the procedure(s) to be completed, including the tooth number and restorative material to be used.

This Ohio State University College of Dentistry form is required for each patient, including "back-ups" and must be signed by the patient and have the patient and candidate's printed name.
Cubicle Assignments
Cubicles will be assigned using the candidate sequential number (located on your CDCA Candidate Indentification/Information Card) by the College of Dentistry. The assignments will be posted the evening prior to the examination on the window outside of Clinic Administration.
NOTE: LEFT HANDED CANDIDATES - PLEASE CALL THE CLINIC ADMINISTRATION OFFICE AT (614-292-6983) ONE WEEK PRIOR TO THE EXAMINATION DATE. WAITING UNTIL THE MORNING OF THE EXAMINATION COULD DELAY THE START OF YOUR EXAM.
Health and Safety Expectations
All candidates are expected to be in compliance with the College of Dentistry immunization requirements that include:
- Rubella: (German Measles): Immunization or presence of serum antibody.
- Rubeola: (Measles): Immunization or presence of serum antibody.
- Hepatitis B: A completed Hepatitis B vaccination series of three injections. Individuals completing the series since December 1997 must have evidence of serum antibody.
- Varicella Zoster: (Chicken Pox): Immunization or presence of serum antibody.
- Diphtheria: Immunization within the last 10 years.
- Tetanus: Immunization within the last 10 years.
- Tuberculosis: A negative skin test within one year of the examination. Anyone with a history of a previously positive skin test must have an evaluation for disease by a qualified nurse or physician and provide a letter indicating a non-infectious state.
Candidates utilizing a Dental Assistant from outside the College must be able to document that the assistant is at least eighteen (18) years of age and that the assistant is also in compliance with the above immunization requirements
Fees
The Clinic Usage Fees, which apply to non-college of dentistry candidates, is $100 per day.
Instrument rental and supplies fee, which apply to non-college of dentistry candidates, is $100.00 per day
All fees may be paid by cashier's check made payable to The Ohio State University only during business hours in the Clinic Admin Office Rm. 1130. Payment is required prior to start of exam.
Electronic handpieces are not compatible or supported at the college.
Additional Fees: X-ray Fees-To secure radiographs, a paid receipt (from the cashier – see "fees" for cost) must be presented to the X-ray Department.

Equipment Furnished

A dental chair, unit, light and bracket tray will be furnished. The units are equipped with a standard six (6) Pin Line Dentsply (Midwest) for the high speed and a standard four (4) line connection for the low speed. High and low speed air pressure is preset to meet Dentsply Dental handpiece pressure requirements. Amalgamators are Vivadent Silimat, Caulk Vari-mix III, or Zenith. Candidates who bring their own instruments must have enough for the entire examination (including manikin heads, shrouds, etc.). We do not provide sterilization services. If the handpiece is not compatible with our equipment, the instrument rental and supplies fee will be required.
Infection Control
Each cubicle is supplied with CaviWipes to wipe down large surface areas of the cubicle such as counter top, dental chair and operator stool. Barriers are available in the Supply Side Rooms. Birex is available for impression material.

X-Ray Facilities
Any candidate may use the X-ray facilities during regular examination hours. To secure radiographs, a paid receipt (from the cashier – see "fees" for cost) must be presented to the X-ray Department, Room 1117 (first floor).
