EFDA Letter of Recommendation Submission

This form is to be completed by the individual who is providing the recommendation for the applicant.

Admission to the EFDA CE program is competitive. Acceptance into the course is based on the quality of the applicants application along with their scores from the prerequisite Tooth Anatomy course. Recommendation letters from current and past supervisors can strengthen the application.
