How to Apply
Applicants must be graduates of a CODA accredited dental hygiene program, have a cumulative grade point average of 2.70 or higher and be licensed or eligible for an Ohio license to practice dental hygiene. Completion of a college mathematics course at the level of OSU Math 1116 or higher with a grade of C- or above must be completed prior to program entry.

Applications are available January 15 for enrollment the following autumn semester. Completed applications are due by May 1.

Admission is selective. Prospective students should submit their completed application, transcripts of all college work, and two letters of recommendation. For current students and recent graduates, one letter should be from the director of the dental hygiene program and the second can be from the clinical supervisor, a faculty member, or a dentist employer; applicants who have been in practice for more than a year can submit two letters from employers or colleqagues. Students who are completing an associate degree program in the current spring semester may submit final transcripts after the May 1 deadline as they become available.
