Background check. Upon admittance, a current FBI background check will be required to begin the program along

Acceptance fee.  Upon admittance, submission of an acceptance fee of $500 that is applied towards the first semester tuition is required.

Tuition and fees. All fees are due one week prior to the first day of classes. Accounts not paid by that date will receive a late payment penalty. For a detailed explanation of fees and tuition, visit registrar.osu.edu.

Immunizations. As a healthcare provider, you will interact with a variety of patients as part of the educational process in the College of Dentistry. Because of this patient contact, and for your own well-being, it is important to have a complete and accurate medical history and immunization record. As a condition of enrollment, the immunization form must be returned to OSU Student Health Services two months prior to the beginning your internship. Your requirements for measles, mumps, rubella, varicella and tetanus/diphtheria must be complete when the form is submitted. The tuberculosis test and influenza vaccine are annual requirements and should be current through the duration of your internship.  If you have this test done at OSU Student Health Services it will be added to your health record; if you have the test done elsewhere you should fax the results to Student Health Services at 614-292-6001.
